Patient Privacy

At Fruitland Crossing Medical Clinic (FCMC) we value patient privacy and act to ensure that it is protected. This policy was written to capture our current practices and to respond to federal and provincial requirements for the protection of personal information. In accordance with the Personal Health Information Protection Act, 2004(PHIPA), a provincial legislation which came into effect on November 1, 2004 and is overseen and enforced by the information and Privacy Commissioner of Ontario (IPC), FCMC will collect certain personal information only for the prioritization of appointment scheduling.

We will only collect the information that is required to communicate with patients. We will not collect any other information, or allow information to be used for other purposes, without the patient's express consent - except where authorized to do so by law. These limits on collection ensure that we do not collect unnecessary information.

Collection of personal information on this website will be restricted to:
-Date of birth
-Phone and/or fax and/or email
-Emergency contact information

As the Health Information custodian (HIC) we will take reasonable steps against theft, loss and unauthorized use or disclosure, and will protect PHI records against unauthorized copying, modification, or disposal
